Dear Parents of Grade 1 Students,

The Demonstration School of Suan Sunandha Rajabhat University has distributed a consent form regarding the services of the Public Health Center 6, Women's Cultural Club, Bangkok Metropolitan Administration. Vaccination services will be provided to Grade 1 students on Friday, November 22, 2024, at the Sai Suwalee Piram Meeting Room, as detailed in the attached document.

We kindly ask parents to prepare the following:

1. Please return the consent form by Monday, November 11, 2024.

2. Please make a copy of the student's vaccination record from birth and attach it with the consent form below.
